Low Platelets with a Low Hemoglobin

Two lines down together is a different situation from either alone, because a small group of conditions tears red cells apart in the small blood vessels while consuming platelets in the process, and those move fast enough that they are treated on the day they are recognized. So the first question is not what is causing this in general but whether the blood film shows fragmented red cells, and that answer should come today.

Why the numbers look like this

Most anemias leave platelets alone and most low platelet counts leave hemoglobin alone, because the two cell lines have separate controls and separate failure modes. When both fall together, something is either affecting the marrow that makes both, or consuming both once they are in circulation.

The consuming version is the urgent one. In a thrombotic microangiopathy, tiny clots form across small vessels; red cells passing through are sheared into fragments, and platelets are used up building those clots. The blood film shows the fragments directly, which is why a film is worth more here than any other single test.

The reassuring counterweight is that the commoner explanations are undramatic: an enlarged spleen holding both cell types back, a B12 or folate deficiency affecting every marrow line, or a medicine doing both at once.

Not being flagged is not the same as normal

There is no threshold on either number that identifies the dangerous group, which is what makes this pattern awkward. A platelet count of 90 with a hemoglobin of 10 can be a stable picture in liver disease or the opening of a microangiopathy, and the figures look identical. What separates them is the film, the trend over days, and whether the person feels unwell.

What else on the report can hide this

The film is the test. Fragmented red cells, called schistocytes, alongside a falling platelet count is the combination that stops everything else and starts urgent treatment. Their absence redirects the investigation toward marrow and spleen.

LDH, bilirubin and haptoglobin add the confirmation, since destruction spills cell contents and consumes haptoglobin. Kidney function belongs here too, because the same small-vessel process damages the kidney and a rising creatinine alongside these counts is a strong signal. In pregnancy or the days after delivery, this pattern has its own urgent differential involving blood pressure and liver enzymes, so a pregnancy status is part of reading the result at all.

What usually causes it

Listed from most to least common — not from most to least serious.

  1. Very common

    An enlarged spleen

    Usually from liver disease. A larger spleen holds back both platelets and red cells at once. Look for a low albumin, abnormal liver enzymes, or a spleen that can be felt.

  2. Common

    B12 or folate deficiency

    Affects every line the marrow makes, so more than one falls. A raised MCV points here, and both are correctable.

  3. Common

    Medications

    Chemotherapy, methotrexate, azathioprine, linezolid and others suppress the marrow across lines. Timing against the drug is the clue.

  4. Common

    Alcohol

    Suppresses production directly, and often arrives with folate deficiency and liver disease as well, so it can produce this pattern by three routes at once.

  5. Common

    A viral infection, including HIV and hepatitis C

    Several viruses lower both lines for weeks; HIV and hepatitis C can do it persistently and are treatable, which is why both are tested for before a low count is called immune in origin.

  6. Uncommon

    Immune destruction of both lines

    Evans syndrome, where antibodies attack platelets and red cells together. A positive direct antiglobulin test with spherocytes rather than fragments on the film.

  7. Uncommon

    Thrombotic microangiopathy

    TTP, hemolytic uremic syndrome, or the pregnancy-related forms. Fragmented red cells on the film, a rising creatinine, and someone who is unwell. Uncommon, and the reason urgency comes first here.

  8. Uncommon

    Disseminated intravascular coagulation

    Occurs in the context of severe infection, trauma or obstetric emergency. The clotting screen is abnormal, which distinguishes it from TTP where clotting is usually normal.

  9. Rare

    Marrow infiltration or aplasia

    Considered once the white count also falls, or when the film shows abnormal cells. This is where a marrow examination belongs.

What is usually checked next

  • Blood film, urgently Looks for fragmented red cells. Everything else waits on this, and it can be done within the hour.
  • LDH, bilirubin, haptoglobin and a reticulocyte count Confirms whether red cells are being destroyed, and how hard the marrow is trying to replace them.
  • Kidney function and a clotting screen A rising creatinine supports a microangiopathy; an abnormal clotting screen points at DIC instead.
  • Direct antiglobulin test Separates immune destruction, which is treated differently again.
  • B12, folate, liver panel, HIV and hepatitis serology, and an ultrasound of the spleen Covers the common and undramatic explanations, which is where most of these end up.

When to seek care sooner

  • Emergency Fragmented red cells reported on the film with a falling platelet count
  • Emergency Confusion, slurred speech, weakness on one side, or a seizure
  • Emergency Fever with both counts falling
  • Emergency Passing very little urine, or dark or cola-colored urine
  • Emergency Pregnancy or recent delivery with these counts
  • Emergency Bleeding that will not stop, or blood in urine or stool
  • Same day Both counts lower than the last test, without an explanation
