A High FIB-4 Score with Normal Liver Enzymes

FIB-4 is not a test you have to ask for. It is a sum done on four numbers you have already had measured, and it estimates something the panel itself cannot show: whether the liver has scarred. Because it uses age and the platelet count as well as the enzymes, it can flag a liver worth looking at in someone whose liver tests have never once been abnormal.

Why the numbers look like this

Fibrosis is structural, and structure is not what enzymes measure. Enzymes report cells leaking now, which is why they can be entirely normal in a liver that was injured years ago and has since scarred quietly.

What scarring does show up in is the platelet count, through portal pressure and reduced thrombopoietin, and in the balance between AST and ALT, which shifts as the liver's capacity to produce ALT declines. FIB-4 combines those two signals with age, since fibrosis accumulates over time.

So the score is doing something clever with ordinary numbers: it reads the indirect consequences of scarring off a panel that was not designed to detect it. That also explains its main weakness, which is that age drives the result strongly enough to produce false positives in older people.

Not being flagged is not the same as normal

The conventional reading places advanced fibrosis as unlikely below about 1.3 and likely above about 2.67, with a gray zone between where further testing decides. Above about 3.25 the case is stronger again. Two cautions apply. The lower cut-off performs poorly over about 65, where a higher one (about 2.0) is generally used instead, and the score assumes the platelet count and enzymes are not being affected by something unrelated, which is exactly what a recent illness or an immune thrombocytopenia would do.

What else on the report can hide this

The platelet count is the component doing most of the work, so anything else that lowers platelets inflates the score without any liver involvement. Check that the count is not falling for its own reasons before treating a high FIB-4 as a liver finding.

Elastography is the natural next step where it exists, because it measures stiffness directly and settles the gray zone that FIB-4 leaves. An ultrasound adds spleen size and liver texture.

The score also says nothing about cause. A high FIB-4 needs the same etiology work as any other suspected liver disease: alcohol history, metabolic markers, hepatitis B and C serology, iron studies and autoantibodies. Finding fibrosis without finding what is driving it leaves the important half undone.

What usually causes it

Listed from most to least common — not from most to least serious.

  1. Very common

    Metabolic liver disease with established fibrosis

    The commonest reason for this pattern. Years of fat in the liver, enzymes that may have settled, and the metabolic markers still present: waist, HbA1c, triglycerides, blood pressure.

  2. Very common

    Alcohol-related fibrosis

    Structural damage persisting through periods of reduced intake when the enzymes look fine. A raised MCV or GGT may be the only other clue.

  3. Common

    Age inflating the score

    FIB-4 rises with age by construction, and the standard lower cut-off misclassifies older people. A higher threshold is used over about 65, and this is the commonest false positive.

  4. Common

    A low platelet count from another cause

    Immune thrombocytopenia, a drug effect, or a recent viral illness lowers platelets and inflates the score with a healthy liver. Check whether the count has its own explanation.

  5. Common

    Chronic hepatitis B or C

    Both scar the liver across decades with enzymes that fluctuate in and out of the normal range. Serology once settles it, and both are treatable.

  6. Uncommon

    Hemochromatosis

    A raised fasting transferrin saturation. Treatment prevents further damage, which makes it worth excluding.

  7. Uncommon

    Autoimmune or cholestatic liver disease

    Autoimmune hepatitis, primary biliary cholangitis or sclerosing cholangitis presenting late. Autoantibodies and immunoglobulins.

  8. Uncommon

    A recent acute illness distorting the inputs

    Infection or inflammation moves both platelets and enzymes temporarily. Repeating when well gives a score worth acting on.

What is usually checked next

  • Elastography, or an equivalent stiffness measurement Measures fibrosis directly and settles the gray zone, which is what FIB-4 is designed to triage into.
  • Repeat the score when well, checking the platelet count has no separate cause Removes the two commonest false positives at once.
  • Hepatitis B and C serology, iron studies, autoantibodies Finds the cause, which the score does not address and which determines whether anything can be treated.
  • HbA1c, lipids and waist measurement Establishes the metabolic contribution, which is the leading cause and the most modifiable.
  • Ultrasound with Doppler Shows spleen size, portal vein caliber and liver texture, and supports or undercuts the score.

When to seek care sooner
